Frequently Asked Questions

The respirator medical evaluation is a questionnaire required by OSHA prior to respirator use within the worksite. A medical evaluation may not require a follow-up exam with your primary healthcare provider unless it is required.
Medical evaluations must be paid by the employer and any follow-up medical visits if it is required.
If an employee is required to use a respirator on the job, they must complete a medical evaluation prior to being fit-tested for a respirator.
The medical evaluation will be reviewed and the employer will receive a medical clearance letter stating the one of the following:
    • If the employee is cleared to wear the respirator
    • If there are any restrictions if the employee is cleared to wear a respirator
    • If the employee is not cleared for respirator use and if a follow-up with their primary licensed health care physician is required
The clearance letter will not contain any confidential medical information regarding the employee.
This medical evaluation does not apply to workers with asbestos exposure as they will require a more in-depth questionnaire called the Pulmonary Function Test.
OSHA requires the medical clearance letter to be kept in the employee's confidential medical file for the duration of the employment plus an additonal 30 years.
